Modeling flow between two bodies

I would like to model flow between two bodies of energy. One will have a higher potential (either heat or electrical potential) and the other a lower. There will be an oddly shaped conducting region that the energy can flow through. How would I model say two potentials (+1,-1) creating a current flow through a non-uniform area, or do the same with two heat sources and determine heat flow. This is basically going to be a method to find "bottle necks" in the conducting region by looking at where the flow is the highest. I will eventually implement this in python with 3D data but for now I really just need to understand what equations I would need to use and solve. I have found ways to solve the heat equation and do this but the methods I have found neglect the "non conducting" regions that I need to take into account. Any guidance would be great.
